Physical Examination

International students with an X2 visa are not required to take the physical examination.

International students with an X1 visa must go to the government designated Qingdao International Travel Health Care Center for a physical examination before applying for the residence permit for the first time. If students did a physical exam abroad, they should accredit the health report (a validity of 6 months) with original copies of all the exam items and also take a required blood test in the Qingdao International Travel Health Care Center.

Materials required for a physical examination:

  • Original passport and copy of passport

  • Three photos (recent two-inch photo with white background)

  • Physical Examination Application Form (provided by the Visa Office A101 and filled out in advance to save time before the physical examination)

  • Original health report and exam items (if available for accreditation to save time and cost)

NB: Do not eat or drink on the day of physical examination

Procedures:

1. Make an appointment for a physical examination

Log into the Qingdao International Travel Health Care Center’s WeChat or official website (http://www.taiz.ithc.cn/sd/) to make an appointment. Kindly arrive on time at the physical examination center according to the date and time of an appointment. If you have not made an appointment, kindly arrive at the physical examination center at 8am on the day of physical examination, otherwise it may be too much for the medical attendants to attend to you due to large number of clients who came earlier.

2. Collect the physical examination report

After all physical examination tests are completed, you will be given a receipt which indicates the date and time of collecting the physical examination report. The report can be collected in person or by proxy. One has to present the receipt to collect the Certificate. Alternatively, students may use the paid delivery service provided by a third party to deliver the physical examination report. Kindly consult the service counter in the healthcare center for more details.

Residence permit can be issued only after the physical examination is qualified and the collected physical examination report is submitted to the Visa office A101. Without a qualified health report, students would not be able to register with the university nor get a residence permit in the Entry-Exit Administration of Qingdao Public Security Bureau, and therefore have to leave China.

Transportation guide:

  • Take the tunnel bus No. 5 / 6 / 7 / 8 at the UPC south gate (University of petroleum south gate) to the Qingdao railway station and then take a taxi at a cost of CNY 25 to the health center.

  • Take the tunnel bus No. 7 at the UPC South gate to Xilingxia Taiping Road, transfer to bus No.312 to Jiangxi Road South Fuzhou Road, and walk about 140 meters to the health center.

  • Take the tunnel bus No. 5 at the UPC South gate to Xikang Branch Road, transfer to bus No.312 at the same station to Jiangxi Road Station of Fuzhou South Road, and walk about 140 meters to the health center.

  • Take the tunnel bus No. 6 at the UPC South gate to Lu Xun Park, transfer to bus No.228 at the same station to Jiangxi Road station of Fuzhou South Road, and walk about 129 meters to the health center.
